We are honored to provide eye care services for our nation's Veterans. Before scheduling an appointment, Veterans receiving care through the Dayton VA Community Care program must first obtain a referral authorization from the Dayton VA Ophthalmology Referral Clinic.

📞 937-268-6511 Ext. 2105 – Request that a referral be sent to Dr. Brian J. Mihok's office for ophthalmology services.
The VA must send referral authorization to our office before an appointment can be scheduled.
Once we receive your referral, our staff will reach out to schedule your appointment.
