Insurance Information
Our practice participates with many insurance plans, including Medicare, BCBS, Tricare, UMR, Mount Carmel, Aetna, Optum, UHC, and others.
Insurance participation can vary by individual plan. Please call our office at (937) 433-0454 if you would like us to verify whether we participate with your specific insurance.
At this time, we are unable to accept Medicaid or Marketplace plans.

Urgent Eye Problems
If you experience an eye injury, sudden vision loss, severe eye pain, or another urgent eye concern, call our office at (937) 433-0454. For after-hours eye emergencies, call the same number and follow the automated instructions to reach the on-call provider.
